Assumption of Risk
I understand that participation in gymnastics, tumbling, climbing, bouncing, and soft-play
activities involves inherent risks. These risks may include, but are not limited to, falls,
collisions with equipment or other children, slips or trips, minor injuries such as cuts,
bruises, or sprains, and serious injuries including fractures or head injuries. I voluntarily
choose to allow my child to participate in Fun Spot Amusements, LLC activities and accept all risks,
known and unknown, even if arising from negligence.
Release of Liability
I, on behalf of myself and my child, hereby release and hold harmless Fun Spot Amusements, LLC,
its owners, instructors, employees, volunteers, and facility providers from any and all liability,
claims, demands, or causes of action arising from participation in any program, class, trial class,
makeup class, event, or open gym. This includes injuries occurring during structured class, during
free play, while entering or leaving the facility, and while waiting in the lobby area. I agree not
to sue Fun Spot Amusements, LLC for any injury, damage, or loss.
Medical Authorization
In the event of an emergency, I authorize Fun Spot Amusements, LLC staff to obtain emergency medical
treatment for my child if I cannot be reached immediately. I understand that I am financially
responsible for any medical care provided.
Illness & Safety Policy
I agree that my child will not attend if they have a fever within 24 hours, vomiting or diarrhea
within 24 hours, a contagious rash, pink eye, or flu or COVID-like symptoms. Fun Spot Amusements, LLC
may remove a child from class if they appear ill or unsafe to participate.
Behavior Policy
For the safety of all children, children must follow instructor directions. Aggressive behavior,
including hitting, biting, or pushing, may result in removal from class. Parents may be asked to
assist or remove their child if necessary. No refunds are provided for behavior-related removal.
Supervision Policy
I understand Fun Spot Amusements, LLC is a structured class program, not a daycare. Parents or guardians
must remain on site for children under age 4, must supervise siblings at all times, and are
responsible for children before and after class.
Photo & Video Release
I grant Fun Spot Amusements, LLC permission to photograph and/or record my child during classes and events.
I authorize use of these images for the website, social media, advertising, promotional materials,
and flyers. I understand no names will be publicly posted.
Cancellation, Makeup & Withdrawal Policy
Monthly tuition is billed regardless of absences, vacations, holidays, or minor illness. Missed classes
are not refunded, but may qualify for a makeup class if notice is given before class starts and space is
available. Makeup classes are limited to 1 per month, must be scheduled in advance, used within 30 days,
and are not guaranteed. To withdraw, Fun Spot Amusements, LLC requires 14 days written notice by email.
No refunds are offered for missed classes, schedule conflicts, change of mind, behavior removal, late arrival,
or early withdrawal without notice.
Sibling & Spectator Policy
Only enrolled participants, assigned instructors, or a parent assisting a child under age 5 if requested by
the instructor may enter the gym area. Siblings are not permitted on the equipment or class floor at any time.
Siblings may remain in the waiting area only if supervised at all times. Babies in carriers or strollers are
welcome, but strollers must be parked in designated areas and no car seats or play mats are allowed on the gym floor.
Repeated safety violations may result in dismissal from the program.
Electronic Signature Agreement
By signing electronically below, I acknowledge that I have read this entire agreement, understand the risks,
voluntarily agree to all policies, and understand that this electronic signature is legally binding.
